An ICU is an Intensive Care Unit in a hospital for critically ill or injured patients who require constant care.  In the twenty-first century; for example in online role-playing games with sexual interactions, I C U means I see you, as in this sexting seems so real that I C U.

I have experienced this for the first time this week, and I can vouch for the fact that the emotions are real.  The fact that sexting is a little creepy adds to the sexual frisson.
